Removing butt implants? 28, 135lbs & nervous my butt will sag (Photo)

Question

I made the mistake of getting butt implants 2 years ago and now I want to take them out because the edges show and it’s a bit too big for my body. I’m nervous that taking them out will make my butt sag. Is there anyway to make sure it will not sag after removal? I’m looking into maybe boosting my collagen or working out prior to surgery. I’m 28 and 135lbs.

More photo angles, including bent over profile and oblique views would be very helpful. Additionally, before photos and more details about your implants is needed (i.e., size, shape, tissue plane “pocket”) before providing much useful feedback. Of course, an in person physical exam would be most useful. That all being said, there exists NO special “collagen boosting” formula/injection that will make any difference other than drain your wallet of hard earned money. Working out after removal may provide some benefit but that depends upon all the above details. On a final note, fat transfer following implants  removal provides little benefit. Glad to help.

Thanks for your question and the photo you provided. For patients wanting to downsize their implants or remove them completely, I recommend fat grafting and skin tightening to the buttocks. This will help prevent that sag you are concerned about. You may also build you buttock muscles with EmSculpt post op to help with your buttock appearance. Based on your pictures, you are an excellent candidate for a simultaneous butt implant removal and fat grafting transfer. This procedure is technical and requires advanced tools and expert technique. First, an ultrasound probe must be used to ensure that the transferred fat does not get into the pocket created by the implant once the implant is removed as this will result in the death of all the fat. Second, use of VASER liposuction to harvest the fat will be required since you probably don’t have much fat to begin with since you elected to have a butt implant rather than a BBL the first time go around. Additionally, VASER allows for thinner layer of fat grafting that is required for refilling of the area where the implant was previously positioned. This is because this layer has been thinned out and can only accept liquified fat that is harvested using VASER technology. Fat should also be harvested with 1) a closed looped system to process the fat in order to optimize transfer time and avoid your fat from becoming contaminated and 2) strategic micro-packet transplant technique that maximizes graft take. These processes have demonstrated over 90% fat graft take. Finally, we must consider whether you will need a skin tightening maneuver such as a lateral thigh and buttock tuck that might be required if you have sagging buttock skin. Last but not least, attention must be given to providing you the buttock shape that YOU desire. This means using our proprietary BBL assessment tool.One of the most common reasons for disgruntled Butt implant patients is that patients are not able to accurately communicate their desired buttocks goals to their surgeons. When gluteal implants are placed above the gluteus muscle at the layer of the deep fat right on top of the muscle it creates a large area of tissue separation.  Damn plant puts pressure on the tissues holding them in their proper place.  normally this area has an abundance of connective tissue fibers that stabilize and anchor the skin and fat layer to the muscle. These connective tissue bands were cut to make room for the implant.

This allows the skin and fat layer to be separate from the muscle layer and when the implant is removed the whole skin and fat layer can sometimes shift position.

Most of these surgeries are done with patients laying on their abdomen and sell the surgeon does not see the impact the gravity has when removing the implant.

Ideally the potential space between the skin and the fat layer and the muscle where the implant used to sit should be closed with sutures and a drain placed.

To minimize the chance of having a deformity that is later very difficult to correct explantation or the removal of gluteal implants needs to be done by someone who has a lot of experience with this procedure.

The providers who seem to have the most experience with gluteal implants all seem to prefer placing the implants intramuscularly or inside the gluteus muscle.

This avoids implant shifting or the ability to see and feel the edges of the implant.

I suggest tracking down one of the handful of surgeons who have a mastered gluteal implant surgery and do a lot of these procedures.

There are a few who answer regularly on real self so maybe one of them will pop up here.

Are usually recommend patients stay local but gluteal implants is a highly specialized procedure that most plastic surgeons don’t have much experience with.  The majority of plastic surgeons in the United States don’t offer the procedure.

There is a reason for that.
