I understand your concern. However, without a proper assessment, it would be difficult to determine what went wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

The majority of plastic surgeons in the United States and to my best guess internationally do not offer gluteal implants. There is a reason for that.

Most gluteal implants (all in the USA) are made out of solid silicone and feel firm especially compared to you Breast implants.

Most likely you were implants were not placed intramuscularly.

Using appropriate sized implants that properly fit inside the gluteus muscle and making sure they are placed intramuscularly is the key to avoiding having implants that show in certain areas and in certain anatomic positions. your concerns and what you described is typical for an implant that was not placed inside the muscle but on top of the muscle and potentially an implant that was aggressive in regards to size.

These are common complaints after gluteal implants and is one of the many reasons plastic surgeons are apprehensive to do this procedure.

In regards to what to do at this point your surgeon is responsible for the outcome of your procedure and your well-being and you are responsible for choosing that surgeon.

Follow up with your provider and ask him or her what the best course of action is. That is the correct and most proper way of dealing with your situation.

If you want a proper second opinion then you should schedule in person consultations with local board-certified plastic surgeons who have extensive experience with gluteal implants. If there are no local plastic surgeons who are experience with this procedure then you will need to travel.

For a second opinion consultations you should always bring proper before and after pictures and a copy of your medical records especially the operative report.

These are available to you from your provider if you request them.

The decision to explant should not be taken lightly.

It can cause the subcutaneous gluteal surface to reposition due to the space created by the implant and gravitational pull.

Most likely the options come down to the repositioning of the implant to a more proper intramuscular position with or without exchanging the implant or explantation.

These are decisions that should be made together with your provider or a chosen new plastic surgeon after proper consultation.
