What can I do to improve this situation with my nipples post breast augmentation? (photos)

Thank you for your question. It looks that your nipple issue is due to an inferior malposition of the implants. Scars can be hidden under the inframammary folds by moving the implants upward a bit with revision surgery. Scar revision surgery may be necessary for the severe parts of scars. Rippling may be improved by moving the implants upwards a little. I recommend fat grafting on the thin tissue areas to make wrinkling less visible.

You have bottoming out, and the common mistake is that you can just cut out a huge strip of inframammary crease and fix it- WRONG! Bottoming out is a pocket problem, not a skin problem- you can make that crease scar as long as you like and cut out as much skin as you like and you will NEVER fix it from the outside. I developed a technique over 20 years ago I call the Original Internal Bra (because there have been many copycats since then). It is a strong, permanent internal suturing technique which actually permanently corrects bottoming out. Today it is the most common revision I perform, at least 5-6 times a week- it works and it lasts! I can't take away those unfortunate scars that your surgeon didn't know any better than to give you, but I can tuck them away in the breast folds so you can't really see them.

You can undergo revision of the scars to move them more into the fold which will also excise some skin to help pull the breasts down. At the same time, you can undergo a circumareola incision to pull the nipple-areola complex lower.

Your nipples are probably in the correct place, but your breast implants are sitting too low and possibly some breast tissue is well. This can be lift by raising the inferior pocket internally.
