36 yo f; Are these too bad? I guess I am eligible for dermal fillers? How long would the effect last? Would it need annual touches? TIA

How long fillers will last will depend on the type of filler and the technique used to inject the filler. Typical hyaluronic acid fillers will last about 6 months. There are more permanent fillers like Bellafill which is actually a collagen stimulator. If injected correctly, the results can last for over 5 years. If you are looking for a more natural ans long lasting improvement, my favorite technique is the AutoCorre HD which uses a hyperdiluted HA filler with PRP. The results from this treatment can last over a years and the results can be cumulative ovet time.

Fillers along the nasolabial fold tend to last 6-8 months.  The help attenuate the fold somewhat but a better option is to lift the cheek via a small temporal hairline incision.  This cheek lift can be done in the office under local anesthesia.

I do not believe it is the place of the doctor to answer the aesthetic question of how bad something is. Since beauty is in the eyes of the beholder, the judgement of whether something is really bad should be left to the eyes of the person herself or himself. Once this is determined by the patient, then an ethical physician has the permission to comment on whether s/he agrees that there is something wrong and, if so, what can legitimately be done about it. 
In this case, if the smile lines truly bother you at this time, they can be quite easily treated with an appropriate HA filler. In general, results may last from 8-14 months, so annual touch ups should be anticipated. Fees depend upon exactly which filler is used and how much is needed. As to whether they need to be treated now, that is a decision for you to make. Should you decide to go forward with treatment, you would be wise to consult with an experienced, board certified cosmetic dermatologist filler expert. Best of luck whatever you decide.

Depending on the filler selected, results will last approximately one year.  Yes, touch ups will be needed annually to maintain your results.

You’re definitely a candidate for a filler in order to treat your nasolabial folds. There are great many hyaluronic acid fillers, which suit the bill. Most AHA fillers will last you anywhere from six months and longer, depending on how your body metabolize them. Touchups are only required based on your requirements.
