Question

I’ve been told by a surgeon in Korea that the shadow at my naso labial fold is caused by sagging. I’ve been given an option to do a surgery but I am not sure if that is the right thing to do at this age. I am only 36 and I am not sure if there is alternative option. Also i asked about fat transfer as well but due to low body fat I am not a good candidat

The prominence of the nasolabial fold comes from the fat of the cheek that rolls down inferiorly and medially creating the fold. You also have a little bit of a marionette deformity. You may benefit from an eccentric lip lift, not a bullhorn lip lift. This does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. This takes 1 hour to perform under local anesthesia and costs $4,000.

Any type of facelift including an endoscopic lift is unlikely to give you any worthwhile improvement.  At your age I would advise against it
