I am 23 years old and have been wanting a hair transplant since I was 15 years old. I have always had a noticeably mature hairline and big forehead.I know many doctors advise against getting a transplant and tell you to take finasteride and minoxidil before. In my case, I don't believe that applies to me because I didn't have a lot of hair in the hairline/forehead region to begin with. I'll attach some of my photos from when I was 14-15 years old and some current ones now.

Answer: Depends on a good plannig.. Lifetime donor supply from safe scalp donor area is between 6-8 K FU grefts.on the other hand NW 6-7 grade hair loss,which young ones with early onset of hairloss may more likely exprience at older ages ,may need more then 10 K grefts for a good density.So finasteride and minoxidil is strongly advised to save the existing hairs at least.On the other hand time flies and we all want to look better especially at young ages and hairs are not vital organs.So to my personal idea;you can lower your hairline but you should consider your future hair loss too and have realistic expactations,And surgeon must have an initial plan but also the long term plan for you regarding your future hair loss individually.And the red flag: You must not spend too many grefts for a small hairline zone..I hope it helps.

I think the key question here is really how much diffuse thinning is present (DUPA) and how much hairloss is likely to occur over the next 80 years. Sure, a hair transplant may look fabulous in a year or two but a wise surgeon is thinking about how things will look in 30 -60 years. I think DUPA must be confidently ruled out here. A transplant at 23 is not without risk. I would be extremely careful about using up too many grafts to bring down the hairline. The photo from younger days is a hairline position that one should not strive to go lower. The male in this photo is a risk for advanced balding and a good plan - both medical and surgical- is needed.
