37, female, under eye hollows - Is lower blepharoplasty possible? (Photos)

Woman 37 y.o. I had these hereditary under eye hollows since childhood with not enough volume. I tried restylane under eyes 5 years ago which improved the area but it gave me Tyndall effect. I still have around 20% of the filler left. I wonder if lower blepharoplasty or fat transfer could help? Or any other procedure? Unfortunately I don’t have PRF or Renuva in my area if that an option.

Full face photos that show the cheeks would have been very helpful in this case.  Since the medial upper cheeks constitute a major support for the under-eyes, when there is volume deficit, either with age or from a genetic predisposition, the under-eyes can be left hollow. If indeed this is the case here, then the use of a robust HA volumizing filler for cheek augmentation would not only create a more feminine Ogee Curve for the cheeks, but would likely secondarily improve the hollowing of the under-eye region. You would be wise to consult with a board certified cosmetic dermatologist filler injection expert and best of luck.

Based on these photos, a transconjuctival lower blepharoplasty with periorbital fat grafting would be recommended procedure. Fat grafts, in particular nano fat grafts, have shown significant improvement of hollowness and dark discoloration of the lower eyelids in our 25 years long experience with this technique.
