Looking to re-inflate my deflated breasts, small cup size increase (small B to large B). What are my options? (photo)

Question

I am 5’5”, 117lbs, 45 years old. I breastfed my babies for a total of 5 years. I would like small implants so that I may increase my small B cup breasts to large B cup breasts. I do not wish to venture into the C cup range. What are my options? Thanks in advance!

It is possible to achieve a natural-looking breast augmentation with small implants and minimal scarring, but an in-person examination is necessary for you and the surgeon to determine the best implant types and techniques.

I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants, and they choose what looks right for them. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. It would be good to create a dual plane pocket through inframammary fold incision and insert an implant of about 250cc. If you want a more natural look and feel and better cleavage, it would be better to do fat grafting as well.

You have 2 problems. One is your breasts are small and the second is your breasts are low on the chest wall. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers or small implants can be placed if additional volume is desired.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

I appreciate your concerns as childbirth and breast-feeding can deflate the breasts. 
  You are correct to be careful in choosing breast implant size for a natural result because you appear to have little or no body fat. However, you do have breast tissue, and your breasts are small but attractive. You can easily obtain a natural result with round smooth implants in the sub muscular plane.​   I do think that 250 mL implant range is  a reasonable size for your body type, breast base diameter, and size desire, but physical examination is always needed. I also like to use the implant in brassiere technique to try different size implants to assist patients in choosing the best size for them. Another nice exercise is finding photographs of breast size that you like, and show them to your plastic surgeon. You have done exceptionally well after having kids and breast feeding.  You would just require implants only.  The approach can be via inframammary (incision in the tuck) vs periareolar (incision around the inferior aspect of the areola).  The implants would be placed partially behind the muscle.

As far as sizing that part is really difficult to answer  as there is a lot that goes into sizing our patients.  We perform measurements, evaluate the skin envelope elasticity, look at photos, look at diagrams and finally use the Allergan sizing system to size the patient in the office.  We obviously listen and discuss exactly what the patient wants and doesn’t want so that we can deliver the expected results.

Your best path to determine what size would work and how big or small you can be would be to seek out a board certified plastic surgeon to discuss your options.  Make certain the surgeon you choose has great deal of experience in breast implant surgeries.
