33 year old with sagging jowls and double chin. Do I need a neck lift or liposuction and/or suture under neck? (photos)

With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). it is hard to tell for sure without an examination. Based on your photos, it looks like you would benefit from chin liposuction. Fullness of the submental area (double chin) can occur from fat deep to or superficial to the platysma muscle. If you animate your face (lower your lower lip) and fan out your platysma muscle you can palpate your submentum and see how much of the fat is superficial to the platysma - this is the fat that liposuction can help. Often times fat deep to platysma may be causing the fullness and in these situations liposuction of the neck may not help it’s fullness while also causing contour abnormalities. Jowls are descent of your facial fat compartments and skin - this will not get better with liposuction.
