From the first visit till now, Dr. Ablavsky and his team have been very professional. I had my Mohs survey done by Dr. Sorace, since I han skin carcinoma on my nose, and then the following day, Dr Ablavsky did reconstructive nose surgery. It was an ambulatory procedure and all went well. This happened two weeks ago, now it’s only a matter of my body to heal. The first thing I noticed about Dr. Ablavsky was that he is a compassionate, nonjudgmental, active listener who provides education as well as feedback in a calm, respectful, and authentic manner. Most impressive is the time he takes with his patients. Dr. Ablavsky does not triple book so that you wait anxiously in the waiting room for an hour or two. Nor does he hurry you through your appointment or appear distracted by his busy schedule. Insurance policies often differ when it comes to breast reduction authorization but most look for a functional deficit caused by large breasts (rashes under breast, bra strap grooves, and possibly upper back pain). The Schnur sliding scale is a chart that will determine how much volume needs to be removed for coverage - volume removed is based on BSA (height and weight metric). Bra sizes are often subjective and can very from one company to another.
Breast reductions risks include but are not limited to infection, bleeding, seroma, hematoma, scarring, asymmetry of breast shape, size, nipple position, incision separation, loss of blood supply or necrosis/loss of nipple, changes to nipple sensation including a numb nipple, fat necrosis, pain, need to have revision or further surgery, cardiac, lung complications inherent to anesthesia as well as blood clots and pulmonary emboli. I make sure to have a thorough discussion with patients regarding risks at the consult and pre operative appointment so that the patient can make a well informed decision regarding surgery. A patient in overall good health with a BMI less than 30, non smoker, can minimize risks of complications but they do still exist.
The technique and scar placement is similar for breast reduction and breast lift procedures. Both narrow the breast, and raise the nipple and overall breast itself into a higher position on the chest wall. Although both procedure reduce the overall volume and weight of a breast, a breast reduction typically removes more than 250 grams of weight from each breast and can be greater depending on the goals.
Insurance plans often have different requirements for breast reduction approval. Some of the more common requirements may include symptoms of upper back/shoulder pain, bra strap grooves, and rashes under the breast that do not improve with dermatologic medications. Additionally insurance companies may want an estimated weight of how much breast tissue would be removed. The Schnur scale represents the minimum amount of breast tissue to be removed (based on your height and weight) for insurance plans to cover the procedure. The larger the body surface area, the more breast tissue needs to be removed. Unfortunately it is not always safe, in terms of nipple areola preservation (blood supply/sensation), to remove the said amount of breast tissue as per the Schnur sliding scale. Always have an in-person consultation with your plastic surgeon to discuss your options.
Some of the risks of breast reduction surgery associated with "dead tissue" and wound healing complications can be attributed to issues of blood supply. Inadequate blood supply to the breast tissue and the skin edges of the incisions results in loss of tissue or poor wound healing. Whether the breast tissue has appropriate blood supply is determined by technique in the operating room as well as certain patient characteristics. Patient factors such as age, smoking, high BMI, diabetes, excessively pendulous or very large breasts, can predispose to wound healing problems.
