I have over muscle silicone medium plus implants 400cc. 4 months ago I had fat transfer to hide rippling between cleavage and to give a more natural look. It’s helped but I can still see rippling on one side of the cleavage. Should I have it done for the second time? Does fat transfer take a few attempts to work? Thanks.

No, since you have large and heavy breasts, they are sagging from the weight. If you add more fat, which is more weight, you will still sag. You need a reduction which includes a lift. with just a lift, you will sag again from the heavy weight.
In most cases I prefer to inject fat simultaneously with implant removal. If implants are removed from below the muscle the breast tissue has already been expanded; fat can be placed above the muscle immediately. The appearance of a two cup size augmentation may be achieved with one session. ...
Placing the implants behind the muscle is usually the best choice to minimize the risk of ripples. A fat transfer can also be done to help hide the ripples.
