Nipples stick to fascia from gyno surgery. How can I fix it? (photo)

Thank you for your question and photo. I would recommend liposuction and fat graft the first instance as this is more natural (your own body) and hence unlikely to get rejected in the long term. I have had good results with fat grafting and correction of this problem. Best of luck. 

I am sorry you are going through this.  It appears that you have fat both above and below the nipple indentation.If this is true, then liposuction along with the use of a pickle fork (a liposuction/fat grafting instrument made by Grams Medical) can stir up some free fat cells and cut the scar tissue binding the nipple down.  The free fat cells can create a non-sticky bed of tissue to allow the nipple to lay down smoothly without denting again.Your plastic surgeon's concept of using a silicone patch beneath the nipple also sounds like it may have merit.  Unfortunately, this is not a simple problem, and you and your plastic surgeon may have to try more than one technique before the problem is cured.

Unfortunately I have seen varying degrees of this on more than one occasion.  It is the result of over-resection of the breast tissue from beneath the nipple complex.  I do not have any experience placing a silicone sheet in the breast in an attempt to correct it.  While it may certainly prevent the deep part of the nipple from sticking to the fascia, my concern would be that it would not correct the absent tissue or the majority of the contour problem.
I have had very good results with using fat grafts and cutting of the scar tissue tethering the nipple down.  Just a warning though, it often requires multiple rounds of fat grafting as you can not place large amounts of fat during any one session.  Ultimately the results have been great.
