Thank you for submitting your question and photos. You may be a candidate for a mini facelift to reduce the skin laxity you’re referring to. This procedure can revitalize your appearance by tightening your skin, removing excess skin, and angulating your jawline.
The results from a mini facelift can last for up to eight years or more. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon. They will be able to discuss your concerns and aesthetic goals with you to determine the procedure that may be right for you.

With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). At the same time, I would recommend lasers (Fraxel, CO2, Halo, or Erbium) and microneedling RF (Fractora, Morpheus8, PiXel8, Intensif) to improve the crepey skin and wrinkles. Overall, the neck can be treated with a combination approach:

1. Microneedling and laser treatments for texture

Whether or not a necklift is an appropriate option depends on a few things that aren't apparent from your photos, and would require a personal examination. For instance, what is the cause of these wrinkles in the front of your neck? Is there a long scar hidden in the wrinkles? Does examination reveal that the skin in that area is stuck down to the underlying fascia or muscle? You have a slender neck, without much subcutaneous fat as padding between skin surface and platysma muscle, which suggests that your skin is adherent to the muscle. This is a difficult problem but possible options include release of any adhesions and fat grafting or placement of a thin biologic graft between the skin and muscle, as well as lifting the neck skin to remove any laxity in the front. However, this all depends on the findings of an in-person examination.

Scarring is not an excuse because the incisions for a lift are very well hidden. You appear to have indications for a lift. Its best to keep looking for another opinion in your area.

Based on these photos and short of in person evaluation, I would say that you are candidate for neck lift. Although you do not have mayor issues like fat deposits and visible neck muscle (platysma) bands, you have skin laxity that needs to be addressed. This can be achieved, in your case, with extended posterior neck lift, where scar is placed behind your ear on both sides. Good luck.

Please remember that a facelift is a great necklift that allows for adequate removal of skin excess, which is present in your pictures. Your request is very common in my practice and my approach is to perform the correct procedure for the individual patient. This isn't always the procedure that the patient requests.

Much more information is needed, such as a full set of facial and neck photographs from all angles in a chin neutral position, along with your age. In our practice, I necklift does not tighten loose and inelastic skin in the neck. The necklift only involves removal of fat deposits above and below the platysma muscle in addition to a platysma plasty. When there is loose and inelastic skin and the neck, then a lower face and neck lift is the appropriate procedure. The skin tone and the neck is the determining factor on which procedure to undergo.
