Asymmetry and square shape years after neck lift. What are my options?

Question

I had a neck lift a few years ago with muscle tightening. I had to go back several time for them to open the scar and do more lipo on my chin area. I feel like my face is asymmetrical and square, which is not what I wanted. I have done lot of treatments such as Infini RF to try and help my face and this issue. I am very self conscious and not sure what options I have available at this point. Any advice would be greatly appreciated. Thanks!

Thank you for submitting your question and photos. You would likely benefit from a traditional facelift to address the concerns you mentioned. During this procedure, fallen tissue can be lifted and the overall face shape more defined. Excess skin and fat in the neck and jowls can also be removed to create a smooth appearance and angulate the neckline.

I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon. They will be able to review your surgical history, discuss your concerns and aesthetic goals with you, and determine the best approach.

This is my expertise and a huge portion of my practice. Asymmetry is normal and expected. Buccal fat pad gives slimming not symmetry. Masseter botox will make you slimmer and make the jawline more loose. Fillers help give shaping and contouring. If you want perfect or close to perfect symmetry you need maxillofacial surgery or implants customized. I suggest seeing an expert to go through all options. Most of my younger clients use fillers to give them shaping especially on the cheeks, jawline, and chin which make the face look more slim and tight; and threads such as PDO or Instalift can build collagen and support the facial shaping and skin looseness over time, slowing down the aging process and given the face a more “snatched” look; and morpheus8 or PiXel8 deep microneedling RF can tighten crepe skin especially on the neck and jawline and is great for long term tightening of the full face, neck and chest; its often used to give the face a slimmer more sculpted look because it tightens the skin and gives lift. an early face lift can be performed as well even in younger clients if they want a different facial shape, maxillofacial surgery with bone breaking or shaving can also be done. For facial slimming a combination of treatments are always needed. Options include: 1. Buccal fat pad removal to reduce fat on the lower face 2. From the photos that you have provided it appears that you would likely need revision of the neck and face lift.  The face lift portion would address the jowling that you have and some of the redundant skin of the neck.  Exploration of the neck would also be necessary as it appears that submental fat is not your problem but rather prominent digastric muscle bands and perhaps some midline platysma muscle laxity.   An experienced face lift surgeon would be able to determine the areas that need addressing.

In order to correct jowels, jawline and perform shaping of the square face, you will need a full face/neck lift. It looks like you already have some preauricular scar? Good Afternoon! If the face wasn't addressed with the neck lift, that may be what's need to eliminate the asymmetry and square shape. A deep-plane facelift for example repositions the deep tissues into a more youthful position. The jowls are corrected, the jawline defined, and the face obtains a more soft-triangular appearance. Overall, the results can be quite nice and last 12-15 years on average. I would seek consultation with a facial plastic surgeon to learn more. Hope this helps!
