A person’s own fat may be used to improve the appearance of his or her body by moving it from an area of excess (or where it is less desired, such as the thighs, hips or abdomen) to an area that has lost tissue volume due to aging, trauma, surgery, certain diseases, birth defects, or other causes.Ideal candidates are in good health, and have excess fat in some parts of the body and too little in other regions. It is very hard to say without photos.  For most people, cleavage entails significant volume in the upper pole of the breasts and in general fat grafting will not supply this amount of volume.  I would recommend an implant with a lift based on your desires.  

When breasts descend they move downward and outwards as well.  Without an examination or pictures it is difficult to provide an answer.  If you have a large sternum, that space cannot be reduced as it is a bony component and your breasts will not move so fat medially.  An in-person exam with a

This question is very difficult to answer without photos and/or a personal examination. The reason is that there are several factors involved. These include exactly how wide your breast bone is which influences how far it is between the inner edges of the breasts. Also exactly what the base dimensions of the breasts are as well has how thin you are and your starting breast volume all influence what may or may not be possible with fat grafting.Fat grafting has come a long way in the past several years and has been used successfully for breast enlargement. Most commonly in breast reconstruction cases. Cosmetic breast enlargement with fat grafting alone has also been used in a (relatively) few cases. For cosmetic breast enlargement, fat grafting has been successful however there are limitations to the maximum increase in volume that can be achieved with each grafting procedure. Many women may require a second or third grafting procedure to achieve the volume they want and/or to correct for incomplete graft take.Although a very promising procedure, at the moment the most consistently reliable method for breast augmentation is with implants. The goals that you are describing are very individualized in many respects, including the degree to which you would like your breast volume enhanced and what may be needed in order to reach that goal given your specific tissues, existing breast volume, and body dimensions.  These are things that need to be discussed in depth during a consultation with a board certified plastic surgeon.  Fat transfer has become a pretty reliable tool for certain things, given the more recent advances in the technologies involved, but it is not best for everything.  If you are interested in pursuing that, I would suggest that in addition to having experience in breast surgery, you find surgeons who also have experience in fat transfer too, so that all of your options can be presented, and the best one for your individual needs can be recommended.  In addition to being certified by the American Board of Plastic Surgery, your surgeon should also voluntarily participate in the Maintenance of Certification program administered by that board. This is the best way to know that your surgeon not only has the best training and experience for the procedure, but that he or she has maintained currency in the certification requirements as opposed to practicing on a "lifetime" certificate which has only been reviewed once at the beginning of his or her career. Want cleavage? Have a breast implant with the lift.  Fat transfers are of questionable value, if any. Most surgeons do not subscribe to fat transfers of any kind especially, to breasts.
