I had 4 treatments of Exilis Ultra on the front and insides of my thighs. Diane Novacek is the Aesthetician that performs the Exilis procedure and she is awesome!!!! Everyone at Dr. Nein's practice is friendly and extremely helpful. I cannot be happier with my results. It's a bit pricey but well worth it. I will schedule more treatments for other areas. Results are real and no surgery necessary!

I had a fleur de lis incision tummy tuck on 4/23/18. After losing 160 lbs, I had a large amount of excess skin and fat that hung down. I also need a breast and arm lift, as well as a thigh lift. I chose to start with the tummy tuck as it seemed to offer the most bang for the buck. The only complication I had was an allergic reaction to dermabond. My results are amazing. I’m very happy with them and my surgeon and have already scheduled my next procedure.

I've had a great experience with Dr. Nein and being someone who obsessively looked at before and after pictures trying to make sure I picked the right Dr, I wanted to write a review. I had an original breast augmentation with another Dr that didn't turn out as I hoped. Of course I wanted the most natural looking breasts I could get and I ended up with an implant that was so high it was only 2 finger width below my collar bone (when I flexed.) It bothered me more and more and I started buying clothes that would hide my breasts. After looking at Dr. Nein's work, I went in for a consultation. (Remember, when looking at before and after pics, look at women whose natural breasts are similar to yours in size and shape so you can realistically get an idea of final look.) I had an appointment with 3 other surgeons for consultation and canceled them after meeting with Dr. Nein and his team. They were very in touch throughout the process as far as follow up, super welcoming and there was no pressure to schedule the surgery. Dr. Nein has a great bedside manor (gentle, warm and knowledgeable) and he seemed to know exactly what he needed to do for Re-augmentation. They stayed in touch after I scheduled the surgery and Dr. Nein called me the night before surgery to see if I had questions. I was so nervous that I was going to make a bad augmentation worse and maybe I should just leave it alone. I'm glad I didn't. Dr. Nein knows exactly how a natural breast should look and I am so happy I did it. My only regret is that I didn't do it sooner. I can't say enough great things about Dr. Nein, his nurse Darla and his entire team. They even sent flowers after the procedure. For breasts, I'll recommend Dr. Nein to everyone. Dr. Nein and his entire staff were so wonderful throughout the entire process of my Mommy Makeover. At my consultation, they answered any and all questions and concerns I had. I called multiple times prior to scheduling my surgery with additional questions, and they were always more than willing to address them. Following my procedure, Darla was so wonderful about calling to check on me daily. I had one issue arise on a Saturday evening, and I hesitated to contact Dr. Nein because it was the weekend. Very thin women with very little breast tissue are more likely to show visible rippling regardless of type of implant. There are a number of options which can reduce that risk and fortunately in most cases camouflage 'visible' rippling. (With very thin women there may always be rippling that you can feel, especially at the lower outer portion of the breast.)First stay away from saline implants. They are notorious for rippling which is why in the majority of cases they are placed under the pectoralis muscle. Placing the implant under the muscle gives some more padding over the implant to camouflage rippling and also the natural muscle tone puts a little pressure on the implant to help 'fill' out the ripples. Placing the implant under the muscle is also an option when using silicone implants especially in small breasted women for the same reasons.Implants also come in different fill volumes and different amount of cohesiveness. The Allergan SRM implants that you have are the softest that Allergan makes. There are two other options the SSM and SCM which are progressively firmer and thus less likely to ripple. The trade off is that these implants are firmer and thus less natural in softness. Implant size is also a factor. The larger the implant the more likely that ripples will be visible. Larger implants have bigger ripples than smaller ones and are thus more likely to be visible. Also a smaller implant with less projection will ripple less.Another option to increase the coverage over the implants is to add a layer of ADM (acellular dermal matrix) to the inside of the breast pocket in the areas where the rippling is most prominent. This is somewhat more expensive and carries a few additional risks and potential for complications.Finally, fat grafting which you've already had done is another (albeit more complex) way to add soft tissue coverage over the implant.Talk to your plastic surgeon (and possibly others as well) in detail about your options. I generally perform breast lifts (mastopexies) at the same time as placing implants (if implants are a part of the overall plan). There is little, in my opinion, to be gained by staging the procedure unless the choice of implants is large compared to the size of the breasts. Pregnancy will of course have an effect on the shape of the breasts. In fact the majority of breast lifts that I perform are on women who have had one or more pregnancies. How much of an effect pregnancy will have on your breasts is hard to predict. There is no 'rule' that says one must wait until all pregnancies are completed before having a lift performed although it must be understood that a revision later may be necessary. I would say that if you are truly unhappy withe shape of your breasts and that can be improved by a breast lift then you should proceed with the lift and be happy with the way that you look until and if you decide to have children. The most important consideration when choosing implant size is if there is adequate breast tissue to cover the implant. Beyond that the specific choice is a very personal decision and many factors can play into the choice. There a re some special considerations regarding long term results and long term sagging with heavier implants. I would discuss these in detail with your plastic surgeon.
One glass two days before this procedure, really should not be a problem. Double check with your surgeon to be sure however.
The MOST important factor in determining which is better is how much tissue you have to cover the implant. If there is not enough breast tissue to adequately cover the implant then under the muscle is the best way to go. However, if there is enough breast tissue then in someone who is athletic, I might suggest that the better option may be a silicone implant placed above the muscle. (I rarely place saline implants over the muscle because they almost universally ripple.) Having the implant under the muscle can lead to implant/breast distortion when the muscle is tightened/flexed and while using your arms can lead to an unnatural appearance. Ask you plastic surgeon for his/her recommendations.
