Swelling moves in the direction of gravity, so it is perfectly normal to see it "move down" after liposuction. If the lower extremities are becoming swollen, then it is important to raise the legs to allow the excess fluid to dissipate. Swelling can also occur in the pelvis and even down near the genitalia. The vaginal labial lips and the scrotum can become very swollen and black and blue but this will all go away. Discuss your concerns with your surgeon so that it can be evaluated to ensure that nothing more serious is developing.
The swelling from the procedure will travel downhill and this includes the private areas. This seems concerning, but it is just water fluid and your body will take it away.

In men it will cause scrotal swelling and in women -labia major.This is due to the fact that normal tissue drainage is 'clogged' up by swollen abdominal area and the groin lymphatic drainage ports which serve both abdomen and genital regions become overwhelmed by 'extra work' they need to do.Normally it settles after 2-3 days without any interventions but wearing support underwear and elevating legs while in bed considered to be helpful. No drainage is normally required but massaging groin lymphatic glands can help.Consult your surgeon for advise.
