As part of a TT surgery, Doctor is going to transfer fat from flanks to fill minor hollows in glutes to round out my boyish butt. Doctor says that if it doesn't all take, he can inject Restylane to augment. Is this commonly done? And are there better fillers for this application?
Answer: Filling of gluteal hollows Allow me to answer your question as best i can without any photos. However, please keep in mind that an in-person consultation with physical exam, or at least a virtual consultation, is first necessary before any legitimate evaluation or final surgical recommendations are made. Buttock implants fill in some of the “gluteal hollow” and thus occasionally provide the added feminine “hourglass” curves that many patients are looking for. However this is mostly only when viewing the postoperative result from the back side . When trying to achieve that hourglass figure viewed from the front side of the body, actual cosmetic hip implants need to be placed either at the same time or instead of buttock implants. In general, cosmetic hip implants are the most reliable, predictable, and permanent option for filling in the hip dips and creating more of that feminine hourglass figure, whether slim or not. On the other hand, fat transfer to the hip area tends to survive quite poorly in most patients and thus produces only short term results (often less than 10-12 months). Injections/fillers such as Sculptra or hyaluronic acids are cost prohibitive (i.e. guaranteed to be the biggest waste of money of your entire lifetime), short lasting and possibly deadly with high volume injections. Others such as silicone/PMMA/hydrogels/Biopolymer/etc are cheap but extremely dangerous and a personal commitment to a life of serious and sometimes deadly consequences. Typically hip implants are best custom designed and created for each individual patient’s anatomy. Because an extremely few number of board-certified plastic surgeons perform this procedure, make sure you seek consultation with a reputable and experienced cosmetic hip implant specialist. The number of plastic surgeons have mastered this procedure or in a small minority. Patient should be highly selective with using providers for Liposuction and fat transfer procedures. Poorly done, Liposuction can leave people disfigured. Most plastic surgeons think they’re better at liposuction than they actually are. I cannot emphasize enough the importance of provider, selection, and properly vetting providers before having permanent and reversible body contouring procedures. To find the right provider, I suggest patients have multiple in person consultations. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patients with similar body characteristics to your owner. An experience plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. Early fat transfer results can look very impressive but you’re not in any way. Represent final long-term results. Always confirm that after pictures were taken at least 3 to 6 months from the date of the procedure. Considering how much variation there is an liposuction and fat transfer skills I suggest patient start the process by having at least five in person consultations before choosing a provider.
