Flaring and droopy tip when I smile - would a nostril reduction surgery or weir excision give me good results? (Photo)

I had 2 rhinoplasty surgeries, one in 2006 and another to correct the horrible job in 2009. I was happy with my revision for so long. However now I’m noticing droopiness of the tip and my nostrils flare a lot when I smile. I have a petite face so it really takes over. What can I do? I was considering a nostril reduction surgery or weir excision. Do you think that would produce the results I’m looking for ?

It is very difficult to have an opinion with a singular up close photo, however, since you're too concerns are a droopy tip, and wide nostrils, based on what I see, your nasal base does not appear to be terribly wide. Please consider re-asking your questions with additional photographs, or consider meeting in person with a rhinoplasty specialist who has a large gallery of favorable before and after photos, along with many positive online reviews from consumers. Hello and thank you for your question. Based on your photograph, you are a great candidate for a revision rhinoplasty. Maneuvers for your surgery would include tip refinement, tip support, tip elevation, among other maneuvers. I would not recommend external alar base incisions. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

Alarplasty will reduce your nasal flare easily. For the droopy tip you will need more evaluation to figure out the cause. It could a muscle pulling it down, too much columella. With the horizontal crease on the upper lip it is likely that you have a strong nasal depressor septi muscle. A little Botox in that area can confirm that. If not then a tip graft can help.
