Revision rhinoplasty - sensation & fake look (Photo)

Question

Hi, I did a surgery in September 2023, the results were not what I expected the doctor shaved down my bridge and narrowed it and lifted my tip too much. My nose looks shorter and rounder than before. I’m considering a rib cartilage revision to rebuild my bridge but - can they also drop my tip a bit? - now my tip is so hard and I’m wondering if the revision will make my nose super firm indefinitely since we’re using rib cartilage? - does run cartilage make it look fake

There are as many different opinions as there are surgeons, so your question will likely garner many different answers. From my experience as a surgeon who performs and specializes in revision rhinoplasties, I will share my opinion. I absolutely dislike rib cartilage. To my eye, it looks fake and unnatural, puts a scar on your body, and the rib does not behave in congruity with your other nasal anatomy. Instead, when a patient presents with issues such as yours -- when they are in need of building up that which taken away in surgery by an overzealous surgeon -- I much prefer deantigenized cartilage. Granted, it requires special -40 degree refrigeration, but in my opinion, it's a worthwhile expense when it means that I can give a patient a much more natural-looking revisionary result.

Hello and thank you for your question. Based on your description you may benefit from Nasal Tip derotation and tip deprojection. This can be accomplished by releasing all the scar tissue and placing extended spreader grafts to derotate your tip, as well as perform nasal tip deprojection at the same time.   I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person.
