My suggestion is to go first a dermatologist for your acnea and scars. This type of treatments from the dermatologist can affect the skin thickness in a better way and this may help your surgery result too.

Is it possible? Yes. but you have to have realistic expectations as to how much smaller it can get. Your surgeon is absolutely correct that you have thicker skin meaning that you will never have a cute small nose like what you might see on social media. You could try something like accutane to try to shrink the nasal skin some but it only will do so much. Beyond that, you will need to have pretty robust cartilage to support a skin that thick to get any kind of definition so you are definitely looking at rib cartilage. The nose can get smaller/more defined but you have to be realistic about "how small" that is and what is involved to get that look.
