Question

I had breast augmentation revision due to vertical flatness under left breast, like a shelf almost. Much less projection than right breast. So basically I went in to have this fix and now the problem is still there. I also had some fat transfer on that breast to have more fullness in the upper part of the breast. I feel like the implant was unable to fully expand unlike the right breast. So my question is…. Can I have a fat transfer to fill that lower portion of the breast. Thank you

Your breasts are wider than the chest wall and too low on the chest wall.  The breasts will continue to descend inferior and laterally.  At this time, I recommend explantation and lift using The Bellesoma Method.  The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.  Later, fat transfers can be performed if additional volume is desired.

Yes, breast fat grafting is a viable option to help fill in this contour irregularities/depressions. I would also recommend an ultrasound to evaluate the implants and soft tissues. Some plastic surgeons have the ability to do this in office (ultrasound).

Fat transfer can be repeated, regenerative medicine allows us to improve the viability of fat cells. You probably have a tuberous breast deformity because the vertical length of the breasts are short. I would recommend you to get a fat transfer enhanced with PRP or stem cells which are obtained from your own fat cells.

Fat grafting won't help. Your implant is contracted and pulling on the lower pole of your breast, creating an irregular, flat surface. You'll need a total capsulectomy and implant replacement.
