I'm 25 years old and I've lost over 100 pounds . I've already have gotten a pubic lift and a extended tummy tuck but I have lost a lot of volume on my vagina lips and my mons pubis is still chubby compared to how flat my stomach is now. I want my mons pubis to be liposuctioned and my vagina lips to get a fat transfer along side with some vagina tightening from the inside . First 5 pictures are of me and the others are inspo pics of what I would want to achieve and how much would it run ?

Thank you for your question and the photos you provided. Congratulations on your weight loss! Based on the severity of loose skin, I would recommend skin tightening to the pubis in addition liposuction. To obtain the labia majora results you want, it would be best to have fat grafting to the area. Not all of the fat transferred will survive, so you may need multiple procedures to obtain your optimal results. Be sure you consult with a board certified plastic surgeon, good luck!

Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person consultation with physical exam is first necessary before any legitimate analysis or final surgical recommendations can be made.

Indeed liposuction of your mons pubis is a good option to better lean it out and harmonize with your tummy tuck result.

Options for cosmetic enhancement of the labia majora typically include reduction of excess/loose skin ("tucking") and/or fat transfer. In general, i have found patients to be more satisfied with the results of tucking. The results of fat transfer are more unpredictable, unreliable and can be uneven and/or leave behind hard painful cysts of scar tissue under the skin. In rare cases a combination may be indicated. Once a physical exam is performed to determine the balance of deflation to loose skin and remaining skin elasticity, then the best recommendation can be given.

I would strongly recommend you visit not just any plastic surgeon (or gynecologist) but one whom is a reputable labiaplasty specialist to confirm all the above. This surgery is probably the number one botched surgery by many inexperienced novice surgeons thinking the procedure is so easy they can just do it without much training or skill. you look like have fat excess on pubic area which is removable with liposuction. you have labia majora atrophy because of massive weight loss and it can be rejuvenated by fat injection as well. it can be decided that if you need a vaginal tightening surgery after a physical vaginal evaluation. you may be a candidate of posterior diamond resection and vaginal tightening with perineoplasty and muscle repairment

Hi, thank you for the pictures. I agree with the fact that you need liposuction of the Mons area to get rid of the bulge . You can have labia fat transfer with a liposuction of a different area as the amount necessary for the fat transfer will exceed the mons pubic area liposuction . Congratulations to your 100 pound weight loss. This is not a small achievement. I have thoroughly looked through your pictures, and I agree there is significant laxity, as well as a protuberant mons pubis. So how do you continue your transformation? I have two options of management which vary slightly, but they can achieve your desired plumpness, and youthful appearing vulva along with a tighter inner vaginal canal. 
Plan A:
Our first procedure whether it’s plan A or Plan B would begin by performing a monsplasty using liposuctioning and contouring the mons pubis to your liking.
The next procedure would be vaginoplasty, which is  designed to tighten the inside of the vagina, reduce any rectocele/prolapse, that may be present, along with lifting, and tightening the opening of the vagina and repairing in an aesthetic manner. 
The next procedure is to address the laxity of the labia majora which would include first a labia majoraplasty or “tummy tuck”of the vulva.  Followed by radio frequency of the vulva to shrink wrap the skin from above in combination with the labia majoraplasty. If at this point you would like more fullness we could do a vampire wing lift, which is using PRP to plump up the tissues from below.
Plan B is exactly the same until we begin to address the laxity of the labia majora. You are requesting Fat transfer to the Labia majora, and I do agree that this could achieve your ideal look. However, to do the fat transfer, I would recommend removing the skin of the labia majora first by performing a labia majoraplasty, followed by fat transfer to the vulva. However, this will need to be done in a two-stage fashion because if I remove the skin and then transfer fat in the same setting this may put too much tension on the suture lines and actually create an a separation of incision lines. Therefore, we would do the labia majoraplasty, let you completely heal and then graft fat from another area of the body to transfer fat and create your desired look. Cost is somewhere between 15-20k.

Great posted photos! I agree lipo reduction of mons pubis with micro and macro fat grafting to labial mounds. But the amount of fat needed will be 100-200 cc's per side so need to lipo other areas. Fee from $6,000+ Best to virtual consult with....
