In 2019 I fully bottomed out my approx 20 yearold implant on right side. Since then , even though it was repaired by recreating the pocket and adjustingy crease back to natural my nipple is at about half the size when it’s erect and unless I’m very stimulated by serious manipulation will it even get hard- it’s literally an inverted nipple grade1 since I bottomed out. Why would I loose nipple sensitivity after bottomed out ?

Your implants are bottoming out. Your inferior pockets can be tightened to raise the implant. If you want, go with a higher profile or go down in size.
You actually may need an internal bra of mesh to give good support. Your tissue may just be too thinned from being stretched by the implants. You also may need to go smaller to have less weight on your tissue. I'm not sure if external stiches will do much for added support.
