Costs vary in every geographic area and even withing a geographic area.   I have no idea of costs in Montana and Arizona.   You need to hope a surgeon with offices in both areas answers this.  They may have an idea.  Best idea is to call a few surgeons in both areas.  Call surgeons boarded by the American Board of Plastic Surgery.  My Best,  Dr C
