How do you measure a unit of Botox in the metric system? Having problems in Italy where they charge by the area.

I moved to Milan over a year ago and here they charge for Botox by the AREA instead of Units. For my crows feet when I lived in Los Angeles, I think I got about 13 units on my left side and about 5 units on my right side. How would you describe a Unit of Botox in the Metric System? So far no doctor has gotten it right in Milan.
