Hi there, I am trying to find out if anything permanent can be done surgically to fix a situation I have when I smile. I have never liked how my eyes become completely encapsulated by bags, both upper and lower, when I smile. I have been suggested to get Botox in the past, but I would rather have something permanent so I do not keep dipping into my savings over time. I feel horrible about myself anytime I see pictures of me smiling. What should be an expression of sheer joy makes me upset.

Answer: Muscle roles underneath eyelashes If you look other people faces when they smile, you will see this happining to all of them. However, in some patients, portion of the orbicularis oculi muscle can be larger (hypertophy, frequent in myops). In some patients it may be considered to shave off a small amount of muscle. This can be decided only on the baisis of in person evaluation, discussion about your goals and risks involved. Godd luck.
