35 year old male. Very fit at 5’8” and 145lbs, but no matter how much cardio, weight is so easily stored on my face. I just want my jawline to be visible, there’s pockets of fat that and I have a bit of submental fat. I’ve already booked a chin lipo + j plasma, but based on these pictures, do you think a neck lift is needed as well? Thanks in advance!

With this anatomy, it is more appropriate to consider liposuction, central platysmal muscle repair and lift (deep neck lift), and placement of a chin implant to "restore" chin volume. This would correct your issue. You may even consider reduction in your submandibular gland volume as it is easily seen when you turn your head 3/4's. I would recommend seeing a specialist in deep neck surgery to get the results that you really want. Liposuction and J-plasma alone simply won't cut it with your anatomy. The prominent contours below the level of the jawline appear to be related more to the digastric muscles as the main contributors, followed by the submandibular glands, and probably a small amount of deep-seated fatty tissue in the central segment. Considering these factors, an anterior neck lift intervention may be more appropriate for achieving the jawline definition you aspire to. This procedure can reduce the volume situated deep to the platysmal muscles, including fatty tissue, digastric muscles, and the lower part of the submandibular glands. Since there is no excess skin, an anterior neck lift would involve an incision under the chin but would not require any skin tightening. This approach is more likely to produce the desired contour and definition along your jawline.

Answer: Would you consider chin implant surgery? Based on your photos, you appear to be skinny, and you also appear to have an underprojected chin appearance. A chin implant may be the best way to improve the appearance of your jawline. It's difficult to tell from the photo if the bulge under your jawline is simply your submandibular salivary gland, or if it is fat.
