To make a quality assessment regarding the outcome of any plastic surgery procedure we need to see proper before and after pictures. If you don’t have before and after pictures and ask your surgeon to forward the pictures they took. For patients who have a flat spot, especially on their lower inner buttocks. I usually inform them the flat spot is likely to remain after a fat transfer and a BBL may not be a good idea. The area is unlikely to project or become more round by grafting fat. The contour is due to connective tissue, and this has no flexibility, so adding fat doesn’t change the contour. The surrounding areas have less connective tissue and expand, making the flat area look even more flat. Your bottom looks like you’ve had a previous procedure. While it’s not flattering, I don’t know if your procedure made it better or worse without seeing your before pictures. I’m usually cautious about doing a BBL when I see a flat area in the back of the buttocks. Flatness and the hip dip area responds well to fat transfer in that area can usually be improved quite easily. Personally, I don’t think sleeping position changes the outcome, though this is often quoted. I recommend you follow up with your provider. If you decide to move forward with a secondary procedure, ask providers to open up their portfolio and show you their entire collection of it before and after pictures of previous patients who had similar body characteristics.
