Lower eyelid retraction. 4 months ago I had deep plane facelift and upper/lower lid bleph. Do I need another surgery?

You are referring to postblepharoplasty lower eyelid retraction. Revision eyelid surgery is possible to lift the lower eyelid. It is important to understand canthoplasty alone will NOT be enough and in fact can distort your eyes. The lower eyelid needs to be lifted called lower eyelid retraction surgery. It also appears you have large eyes which predisposed you.

You have some scleral show on both sides, more on the right.  You also appear to have large eyes.  It would be helpful to see where the lids were pre-op.Fixing this is not an easy revision.  It can sometimes require a graft in the lid, and can change the shape of the eye.  This is especially true in someone who has prominent globes.  I would wait at this point to at least 6 months post-op.  Some massage upward on the lid may help.    

When your lids don't close completely you always worry about dry eyes and corneal ulceration.Perhaps massage and taping especially at nite to make sure your ids are closed.If this does not improve you may want to get some tightening to your lower lids.

I only see minor scleral show. Otherwise the results appear very good. You might be having an episode of unrealistic expectation. Best to see your surgeon in person to discuss all your concerns. 

We see regularly see patients who have undergone multiple facial procedures and eyelid surgery through an external approach under the eyelashes and present with lower eyelid retraction.  In your photo, you can see inferior scleral show. This is the white of the eyes.  This is best repaired by a fellowship-trained oculoplastic surgeon in your area. I would refer you to the ASOPRS website.  This can be addressed through the placement of a spacer graft inside the lower eyelid to help lift up the eyelid to a more natural appearing position.   There are several excellent surgeons in the Miami area. I would suggest you see Dr. Michael Connor in Palm Beach.

From the picture you posted it appears both lower eyelids are pulling down although the right one is worse. I would definitely use artificial tears during the day and sustained ophthalmic ointment at night to help with the irritation. At this point since it is not severe I would hold off on additional surgery, waiting at least another 3 months to allow it to improve on its own unless it is causing any exposure problems to the cornea(you would need to see an ophthalmologist to check for this, I suspect that eyelid may not be closing all the way at night based on your symptoms).
