I'm 60yo 160cm tall 144 lbs in good health. I have banana rolls and a flat sagging butt. Ideally I would like Butt Augmentation but in the UK BBL and Butt implants are not often performed due to difficulty getting insurance re BAAPS concerns over safety. Would skin excision improve the look or make it worse? Any other treatment suggestions?
Answer: Skin excess and laxity Thank you for your question and attached photos. It is difficult to asses your degree of skin laxity and excess based on these photos, however to address your banana rolls/lower buttocks I would recommend skin excision in this area. An alternative option includes RF skin tightening to this area, however there appears to be a significant amount of skin excess, so skin excision would provide the most significant improvement. This procedure could be combined with BBL (fat transfer) or buttock augmentation with implants in order to give you more buttock projection. Make sure to consult with a board certified plastic surgeon, good luck!

Answer: Treatment for lower buttock “wrinkly skin banana rolls” Buttock “sagging” is usually a combination of loss of volume or deflation and/or the effect of aging and gravity. So, I personally first treat patients like you by restoring volume to the buttock with a buttock implant. This will create a more desirable smooth and rounded perky buttock profile. If not adding volume with an implant, the buttock will be shortened but may appear more flat with just a buttock lift. A lower buttock tuck is usually then performed at six months following implants. In this sequence, the implants have fully settled into final position so that any remaining loose skin can be most accurately identified and removed with the lower buttock tuck. Please note, because an extremely few number of board certified plastic surgeons specialize in buttock implants, you may have to travel away from your local area to seek consultation with this particular type of surgeon. Answer: Wrinkly banana roll It appears that the primary problem is a lack of skin elasticities combined with a lack of volume. Did you have Liposuction in this area any time in the past? Considering that the problem exists because of both skin laxity due to a loss of skin elasticities and volume depletion treating either of those should give some improvement. Exciting skin will leave a significant scar. The BBL procedure or grafting fat in this region can be done but the results vary substantially depending on who does the procedure. In your case I’m not sure I would even call it a BBL but rather restoring lost volume. It’s very important to know if you’ve had previous procedures such as previous Liposuction in the area.
