Would sliding genioplasty to advance/lengthen my lower jaw improve my profile? Can I combine with lower face/neck lift? (Photo)

Question

I've hated my profile all my life. I was tormented, called "frog" and even physically assaulted constantly as a child. It's affected my life dramatically. Even now, at 57, I turn my head if I suspect someone is looking at my profile. I recently had a successful upper blepharoplasty (review coming soon ), and plan to continue my journey with dorsal graft rhinoplasty and face/neck lift. My chin is short and recessed, my jawline very weak. I'm leery of an implant. Any advice would be appreciated.

Hi Mirage511,I'm sorry to hear about your difficulties.  Your chin can be improved indeed, although an implant may be just as good as a sliding genioplasty (both involve foreign material and I do both frequently).  However, you are also someone who may benefit from augmentation at the base of the columella. this is where the bottom of your nose in the center meets your upper  lip.  This is somewhat recessed on you and could quickly and easily be adjusted to give a beautiful profile.

Despite how you may feel about your chin, it is not tremendously deficient. But to avoid an implant, a sliding genioplasty can be done to bring it forward and provide a little vertical length. This will have the effect of lengthening your lower jaw and will work well with any lower facelift procedure. The sliding geniplasty can be done at the same time as the lower facelift.

A sliding genioplasty is performed by oral surgeon under general anesthesia in a hospital setting with an overnight stay. This involves making cuts in the jaw bone and  setting it forward with plates and screws. This procedure is not combined with any other cosmetic procedure.  A chin implant can be placed at the same time as a facelift through the very same incision underneath the chin which gives the added augmentation required to balance the profile. Chin implants are composed of Silastic, are manufactured in a large variety of sizes and shapes, and are very well accepted  by the human body. We have been performing chin implants for 25 years with an excellent success rate. A lower face and neck lift accomplishes tightening loose facial and neck skin, lifting the jowls, tightening loose facial and neck muscles, and removing the fatty deposits in the neck. You should rethink your interest in genioplasty.There are lots of permanent metal objects, (wires, screws or plates) used to hold the bony slices in place. You cannot easily revise a genioplasty. It has more morbity, more cost, more pain, general anesthesia and more downtime.You may also need a maxillary augmentation.
