Nipples leaking fluid 9 years post breast augmentation - what should I do? (Photos)

I got my breast augmentation in 2018 in Medellin Colombia by Juan Diego Mejia Jimenez. i got 425 ccs gummy implant placed under the muscle. Ive been pregnant twice since my surgery in 2018 My first pregnancy i had no complications, my secong pregnancy my nipples started leaking clear fluids. At first the leaking would come and go but now there’s more fluid. The fluid is clear/slightly yellow and it makes my nipple stick to my bras and pulls the skin off the areola. What should i do? I got my breast augmentation outside of my country and to my knowledge , the surgeon who performed on me no longer does breast augmentations.

Clear or yellow nipple discharge years after breast augmentation should be evaluated in person. It may be related to hormonal changes, pregnancy/breastfeeding effects, duct irritation, infection, skin breakdown, or less commonly an implant-related problem.
Because the fluid is increasing and sticking to the bra enough to pull skin from the areola, I would not ignore it. You should see a breast specialist, gynecologist, or plastic surgeon for an exam.
The evaluation may include breast ultrasound, mammogram depending on your age and history, possible MRI if there is implant concern, and sometimes testing the discharge. It is also important to check whether the fluid is coming from the nipple ducts or from irritated/cracked areola skin.
Seek urgent care sooner if you have redness, warmth, breast pain, fever, swelling, a new lump, bad odor, pus-like drainage, or sudden breast shape change. These could suggest infection or another problem that needs prompt treatment.
My advice would be to arrange a local evaluation rather than trying to contact the original surgeon abroad. Bring any implant information you have. Most causes are treatable, but persistent or increasing nipple discharge should be properly checked before assuming it is normal after pregnancy.

Thank you for describing this so clearly, and I want to gently encourage you to have this evaluated in person soon &mdash; not because I want to alarm you, but because new or increasing nipple discharge always deserves a proper workup. The most reassuring news first: nipple discharge after recent pregnancy and breastfeeding is very often benign and hormonally driven, and your implants under the muscle are usually not the cause of clear or yellowish fluid. Galactorrhea (milk-related discharge) can persist or restart for months after breastfeeding.
That said, the responsible next step is to see a breast specialist or your physician for an examination rather than to assume. Because the discharge is increasing, comes from the nipple, and is affecting your skin, I would want you to have a clinical breast exam and, depending on your age and findings, possibly an ultrasound or mammogram and simple blood tests (such as prolactin and thyroid levels). These tests are quick and will sort out whether this is a hormonal issue, a benign duct problem, or something that needs treatment &mdash; and they are reassuring far more often than not. The skin irritation you mention from the fluid sticking can usually be managed with a soft breast pad and gentle skin care once the cause is identified.
You do not necessarily need to return to your original surgeon, especially since he is abroad and no longer doing these cases &mdash; any qualified plastic surgeon or breast surgeon can evaluate you and order the right tests. Please do not wait on this one; getting it checked soon will give you answers and peace of mind.

The fluid leaking from the nipples may not be related to your implants.  I encourage you to begin by discussing your concerns with your OB/Gyn who may suggest some tests to rule out any concerns.  You can also visit with a board certified plastic surgeon near you for an exam.
