Severe nipple pain and spasm in left breast 6 years after Augmentation and lift?

On and off I’ve had episodes of severe nipple pain that last weeks and go away. (On and off for 6 years) But this time it’s been around for 3 months and I’ve been having spasms in my left breast. They are under the muscle and I also had a lift. I’m concerned because as it’s so unbearable I can’t even walk it hurts my breast so much. I also have spasms in my upper arm Is this a nerve or muscle problem?
