You have excess pubic skin and fat from your weight loss in addition to a significant penoscrotal web. I have published extensively on the surgery to correct the buried penis in the plastic surgery literature. You have a relative buried penis. The idea of the surgery is to stabilize the penile skin to the penis, so that it cannot bury. It usually involves removing the pubic fat, excising some pubic skin if it is drooping, and suturing the pubic skin to the underlying muscles. You will also need tacking of the skin on the underside of the penis to the penis at the junction of the penis and scrotum and removal of your penoscrotal web.
