The scrotum is normally attached to the penis; some guys have a low attachment and some have a higher attachment (such as yourself). The Prometheus procedure creates collagen tissue which has a degree of firmness even when flaccid. Therefore, it would definitely decrease the "buried" look.

Fat grafting to the penis has recently become popular in some parts of the country. A study showed poor cosmetic and functional result after irregular resorption of grafted fat. The external appearance of the penis remained stable for months, even though later biopsy revealed all the grafted fat to be dead. The operation, although technically easy to perform, has many serious potential side effects that should be understood.

You have excess pubic skin and fat from your weight loss in addition to a significant penoscrotal web. I have published extensively on the surgery to correct the buried penis in the plastic surgery literature. You have a relative buried penis. The idea of the surgery is to stabilize the penile skin to the penis, so that it cannot bury. It usually involves removing the pubic fat, excising some pubic skin if it is drooping, and suturing the pubic skin to the underlying muscles. You will also need tacking of the skin on the underside of the penis to the penis at the junction of the penis and scrotum and removal of your penoscrotal web. The surgery is relatively non-invasive, but very few physicians know how to correctly do the surgery. It is highly successful.

If you are concerned about excess skin, an abdominoplasty reshapes and tightens the area. Loose skin can be difficult to target with diet and exercise alone. You would also likely get a great result from penoscrotal webbing excision which can give the appearance of an increase in length as well. This procedure is where we remove the web extending from the scrotum to upper shaft of the penis that creates the undesirable turkey neck appearance that men complain of while also displaying more of the shaft to create the perception of additional length.

The fat in the pubic area and the scrotal webbing are definitely making the effective length of your penis shorter both visually and functionally.  The pubic fat and drooping skin could be corrected relatively easily with liposuction and skin lift.  At the same time the scrotal webbing can be excised and a plastic surgical procedure called Z-plasty should be done at the same time to prevent a "bowstring contracture"
Your treating surgeon will most likely be familiar with this procedure but just in case you should mention it to him because a bowstring contracture will be more difficult to correct.
