I’m looking for some advice on implant sizing for a breast revision. I’m 158cm and 50kg, with a breast base width of approximately 11.5cm on one side and 12cm on the other. I currently have implants and have developed a waterfall deformity with significant sagging, so I’m expecting that I’ll need a lift/mastopexy as part of my revision. I’m hoping to achieve a very full, round and projected result rather than a subtle/natural look. I’m considering approximately 450–500cc, ideally around 475–500cc, but I’m wondering how realistic this would be with my relatively narrow breast width. I would be willing to consider an extra-high-profile implant if that would allow me to achieve the volume and projection I’m after without the implant being excessively wide. For anyone who has experience with similar anatomy or surgeons who specialise in revision surgery: What do you think would be the biggest limitations to getting around 475–500cc on a 11.5–12cm breast base? Would the implant diameter, existing pocket, skin/tissue laxity or the lift be the biggest factor?

With an 11.5–12 cm breast base, 475–500 cc is possible only in selected cases, usually with a high or extra-high profile implant. The limitation is that the implant must not be too wide for your breast footprint, or it can create side overflow, symmastia risk, thinning tissue, rippling, or an unnatural edge.
In your case, the biggest limitation may not be base width alone. Because you already have waterfall deformity and sagging, the skin envelope and lower pole support are very important. A large implant can give fullness, but it also adds weight and may worsen recurrent sagging if the lift and pocket are not strong.
The existing pocket matters as well. If the pocket is stretched, too low, or too wide, simply placing a larger implant can make the problem worse. Revision may require pocket repair, fold reconstruction, internal support, or mesh/ADM in selected cases.
A lift can reposition the nipple and tighten the skin, but it has limits. Very large implants combined with a lift place more tension on the incisions and breast tissue. This can increase risks of widened scars, bottoming out, delayed healing, and recurrent waterfall deformity.
My advice would be to choose the implant by exact dimensions, not cc alone. Ask your surgeon to compare base width, projection, pocket control, and lower-pole support. If your goal is very full and round, an extra-high-profile implant may help achieve projection without excess width, but the long-term success will depend most on tissue quality, pocket repair, and the strength of the lift.
