Will cheek reduction get rid of my puffy cheeks? (Photos)

Hi Dr’s I was wondering if cheek liposuction will help get rid of the puffy cheeks. I’m 33 years old, I’m 5’9 in height, very healthy and lean (Skinny) with a good BMI range. I’ve had previous cosmetic surgeries, such as buccal fat removal 4 years ago and v line surgery 3 months ago now but I’m still very self conscious of my big cheeks

Cheek liposuction only helps when the fullness is from pinchable superficial fat in the lower or lateral cheek/jowl area. It usually will not improve midface fullness, facial bone shape, muscle bulk, thicker soft tissue, or swelling after recent surgery. Since you are lean and already had buccal fat removal plus V-line surgery only 3 months ago, additional reduction now could create hollowness, contour irregularity, or an older look instead of simply making the cheeks smaller.
I reviewed the photos; they show persistent mid-to-lower cheek fullness with an otherwise lean face and defined jawline, but the exact tissue layer cannot be judged from photos alone. I would usually let the V-line surgery settle longer, often closer to 6-12 months, then have an in-person exam to decide whether any very conservative lower-cheek/jowl liposuction, skin tightening, or no further reduction is safest.

Malar prominence laterally with relative deficiency in forward projection. Rather than further fat removal, options such as malar (zygoma) lateral reduction may help narrow the face, while a temporal lift (Endotine) can reposition descended midface soft tissue. In selected cases, an upper gingivobuccal approach can be used to address midface contour more precisely.
