You had a protruding tummy prior to surgery so it’s not surprising that you still do. This can be the case even with muscle tightening

Your tuck was able to tighten your abdomen a good bit. It will not remove excess internal fat. Your breasts were augmented but not lifted. An examination is needed to see how your implants are sitting. With smooth implants, it is no problem if the implants flips. You will have permanent numbness in your abdomen from the tuck. Please discuss your concerns with your surgeon.

I'm sorry that you are having issues after your surgery. I encourage you to speak with your surgeon about your concerns and/or visit with another board certified plastic surgeon for a second opinion. It appears that either there is excess visceral fat, which cannot be addressed through a tummy tuck or liposuction, or perhaps further muscle tightening is needed. The breasts will need to be examined for an opinion on implant position as well as the scars.

Since your breasts are sagging, you also need to undergo breast lift surgery. Rectus muscle repair (plication) is also required during abdominoplasty.

Based on your preop photos, you had a pretty significant rectus diastasis (muscle separation) in your abdomen and breast sagging. You less than ideal tummy tuck results may be due to inadequate muscle tightening or it is also possible that you had a significant amount of visceral fat ( internal abdominal fat that's grounds your organs). Visceral fat can't be addressed surgically. Only weight loss will reduce it.Regarding your breast augmentation results, it seems that you would have benefited from a breast lift at the same time or before the implants were placed. I recommend you voice your concerns with your surgeon.
