What would help my protruding philtrum? (Photo)

Thank you for your pictures and your question.  There are two structural reasons why you may be having this - one is soft tissue, the other is skeletal.  The most common is usually skeletal, meaning how the teeth sit (this includes the maxilla and dental tooth complex).  Think of having a curtain hanging down, but you have a window sill that protrudes far out forward from the wall - this is going to push the curtain forward.  The solution in this case is to fix the window sill, or address the teeth/maxilla.  The other reason may be you just have a thick curtain.  In these cases you can do a orbicularis release but that has significant possible side effects - conversely you can also to contour of the muscle which again has its own issues.  if it is only the muscle that is bulky, you can always do a little botox to relax it.  however this will lengthen your philtrum.  ideally I would recommend seeking a consultation to go into more detail about what all of your options are.  Hope some of this helps! 

The most likely reason for your philtrum to protrude the way it does is the underlying bony anatomy of the maxilla/teeth. I would recommend seeking a consultation from an oral surgeon in addition to a lip surgeon.
