I’m scheduled to have a BL/BA. I’m currently a 36C and was wanting a 350cc high profile implant. Unfortunately, the largest implant my PS will allow is 300cc. He never elaborated as to why this was the case. I’m concerned that I’m going to be disappointed with my breast size after surgery. I read so many other reviews from other women who have had a BL/BA with much larger implants then I’m wanting. Why is it that my PS won’t allow anything bigger?

Answer: Why won't my surgeon go with a bigger implant? Without knowing the logic behind your surgeon's decision, I really can't tell you. There are definitely benefits in going with a smaller implant in patients requiring a lift since the smaller implants will be less heavy and potentially allow for more perkiness immediately as well as greater long term durability.If your surgeon won't give you a reason, I would get a second opinion. Answer: Lift instead of implants At size 36 C, you already have a significant amount of breast tissue and you will not notice the difference between 300cc or 350 cc of additional volume. I always recommend the smallest implant or no implants. If you are having a lift that means your skin in unable to hold your breast is the proper position. Adding the weight of implants will make the situation worse. The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation and the ability to breast feed are maintained. At the same time or later, fat transfers can be performed if additional volume is desired. Implants are not lifetime devices and half are removed by 9.5 years. Fat transfers are permanent and incorporated into the body.Best Wishes,Gary Horndeski, M.D.

The size of the implant, your chest measurements, and your desired result are some of the factors should determine what size implant would be best for your body. Some surgeons prefer not to place large implants. I take into consideration what the patient desires as well as what is possible based on your current breast size, amount of tissue and skin, and overall proportions for your body. If you are concerned with the current choice of implants, I would ask to visit with the surgeon to ask why this choice was made and possibly seek another opinion. I wish you luck.

We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process.
