The undereye area is delicate and unforgiving. We are seeing patients who have had fillers years before now presenting with malar edema (swelling on the cheek bones.) In a patient who has had filler dissolved under their eyes, my preference is to treat them with their own blood processed to become an injectable gel. At least from the photos provided, there appears to be some volume deficit in the upper cheeks. If this is indeed so, then the use of a robust HA volumizing filler for augmenting the upper cheeks region (an area responsible for the maintaining a smooth transition between the upper cheeks and the under-eyes) would help to diminish the hollowing seen there and to smooth the transition between the upper cheeks and the infraorbital region.
