Our pediatrician just said to watch it. It is even more noticeable when he's tired or smiling. It doesn't seem to affect his vision and his eyes move together. He was born with the smaller eye shut and it didn't open for about 30-45 min after birth.

Definitely take him to a pediatric ophthalmologist for evaluation. There are a variety of causes of eye asymmetry in babies. Some can have visual impact.

However you baby is clearly using both eyes and the pupil is not blocked by the eyelid.  I think it is very reasonable to have this issue looked at by a pediatric ophthalmologist.  Your pediatrician can recommend someone.  Provided this is small amount of congenital upper eyelid ptosis and it does not interfere with vision, generally the best thing is to do nothing until he is in his teens.  Follow the advice of your pediatrician and pediatric ophthalmologist.
