I have just had an explant of breast implants and fat transfer from my tummy and love handles (3 days ago) am I able to sleep flat or on my side? Sleeping upright is killing my back and neck.

I know it's hard to be patient, but you are much too early in your recovery to judge your results. Because you had conservatively sized implants in for a short time you should have good skin retraction. If after several months you are still unhappy you can visit with your surgeon to explore...
From the submitted photos you will have a better result if you have a lift when you have your implants removed. You are already a candidate for a lift and with the loss of the implant volume the breasts will appear more saggy and flatter.
