I saw a video on YouTube where she says that one of the best ways to minimize nasolabial folds without looking like a bullfrog is by injecting a small amount of filler at the base of the nostrils. I’ve never seen this before. Would you agree with this method? And if so, how much and what kind of filler would one use for this? I already get cheekbone injections that create lift but I still have some folds I’d like to eliminate but I don’t want to look too full at the mouth.
Answer: Yes but be carful I think want you are referring to is injecting into the piriform recess. It depends on the severity of the fold but you can certainly start there. Just be careful if the angular artery crosses superficially in that area so you need to inject deep a cannula is also safer specifically 25G cannula reduces bruising and less risk of compromising the artery and causing an occolusion. Nasolabial fold is high risk! so make sure you go to expert injector who really respects the anatomy. Since this is such a dynamic area of movement depending on the depth of the fold I would use medium G' hyaluronic acid dermal filler or even medium high dermal filler...Voluma, Restylane Lift sometimes even Radiesse if the fold is severe. .1cc-.2cc. Talk to your injector make sure you don't get overfilled in this area as well.

If a doctor is simply injecting the area of the folds, it very difficult to improve them and still have it look natural. You usually have to treat the support structures of the face by injecting in the mid face or cheek area to lift, rather than injecting directly into the nasolabial fold. Each face is different, and the treatment and fillers used are going to depend on your skin type, what other fillers you’ve had, your age and so forth. It is always best to treat the face as a full unit, and think of it that way rather than looking at specific lines, because that is a way of keeping a treatment looking very natural. I prefer to use Sculptra, Juvederm Voluma and Radiesse to lift and shape the face which can improve naso-labial folds and fold under your lower lip rather than simply injecting thinner fillers directly to the folds. Ultherapy and Thermi technologies can also be effective, minimally invasive treatment options for improving marionette lines by lifting and tightening the face.
