Hi there. I had my explant 6 weeks ago. I’ve noticed lumps and bigger areas of hardened / firm tissue in the lower pole. I know the doc left the capsule in. He said it was very very thin like cellophane wrap and he didn’t take it out. I wonder if what I’m feeling is capsule. Ugh. He said it would dissolve on its own? I’m hoping for Fat Transfer in the near future. ... I’m wondering what you think this odd tissue “could be” .... could it effect the fat transfer ?

As you already are planning to have the implants and capsules removed your surgeon can send the fluid away for testing at the time of surgery. There is no specific need to have the fluid aspirated or tested beforehand.
Since your implants have been in place for 18 years it is likely that the breast tissue has thinned during this time and that you'll be smaller than you were before implants. Your surgeon may be able to better estimate what you have in terms of native breast tissue after an examination.
This question is best answered by your surgeon as opinions will vary. Typically you should be able to lay on your side or stomach after surgery as comfort allows. This may vary based on the details of your surgery and how you're progressing through recovery.
