Hello, I had fillers injected to smooth out lines between my eyebrows and above nose bridge about 3 years ago. I recently noticed that two hard lumps started forming near where I was injected. What could they be and how can I get them removed?

Answer: Recommend dissolving Filler Longterm filler problems include migration, edema, and nodularity. Filler migration and permanence is a very common problem because there are many different types of HA fillers on the market and each is specified for a specific part of the face. Use of highly cohesive fillers on thin skin may cause swelling and nodularity. If patients have filler migration or nodules, we recommend a combination approach of filler reversal with enzymes Hylenex and Hyaluronidase in different strengths to dissolve the filler. We may also need to add Kenalog to the mix in order to reduce fibrous tissue. If the filler does not dissolve, it either means the filler is not HA material or we need more enzyme. The HA reversal can be combined with other lasers and skin treatments at the same time, and it usually (usually) does not affect adjacent tissue. Best, Dr. Karamanoukian Realself100 Surgeon

These can happen in months or even years later. You didn't state which filler was used. This can be critical to what treatments may be appropriate. Permanent fillers, such as silicone or Bellafill, or semipermanent fillers, like Radiesse, can be especially difficult to treat as there are no dissolving agents for these fillers. However, I have personally had success in dealing with late complications, such as these complications, using an algorithm of varying combinations, concentrations and amounts of intralesional anti-inflammatory agents. HA fillers, on the other hand, can be dissolved more easily with the enzyme hyaluronidase. You would be wise to consult and experienced, board certified cosmetic dermatologist. Best of luck.

I've seen in my practice a lot of similar cases of patients who been referred to me with concerns of having lumps after dermal fillers or fat injected into the Glabella area. To treat these lumps we should est we should firstly establish (if possible) what type of dermal fillers have been injected! So, If it was, (and most likely hyaluronic acid) it will not be difficult to treat with injection of Hyaluronidase combined with a steroid. It will be much more challenging to treat if they used permanent fillers such as calcium based fillers or long-lasting fillers such as Ellanse as those types of lumps would need to be surgically removed by a facial plastic surgeon.Mr Ayham Al-AyoubiFacial Plastic and Laser surgeon
