I think that nonsurgical modalities are a great tool for the right patient. Filler adds volume. If you are worried about sag and poor contour I do not think that adding volume will make a great difference. Nonsurgical modalities long term are more expensive and require maintenance. For the right patient they are worth while. However, it depends on the degree of improvement you are looking for.Surgical options for a neck have to address the fat below your skin. The muscle laxity. The deep structures under that muscle which include the submandibular glands, the digastric muscle and the fat between them (interdigastric fat). I believe that a board certified plastic surgeon can help guide you in this process. The procedure I described above can be done through a small incision below your chin. A facelift should be used to address the deep nasolabial folds and jowling that you develop with age.
