Turning 40 this year and feel like my face (especially eyes) have aged a decade nearly overnight. I’ve had Botox and dysport before. Never filler (too scared about filler migration) but wondering if upper/lower Bleph will help with the pitosis as well? I often get told I look really tired even after a full sleep.

I would suggest the eyelid surgery just based only on your two photos and not exam. Your brows naturally sit low on your forehead and if raised, your looks will be significantly changed. Not everyone likes that look they never had. The blepharoplasty will look very natural once healed.
To make an interpretation, it would be very good if you could provide a full view of your face (not just around the eyes) from the opposite, right and left sides. It will be more accurate to evaluate by looking at the whole face
Your blepharoplasty is pulling down your eyebrow. Further blepharoplasty will only make this worse.
You need a browlift to restore the previous height of your eyebrows.
