I’m a 44-year-old woman looking into eye rejuvenation options. I currently do Botox around my eyes and forehead but am not interested in fillers. I’ve noticed more of a hollowed, sunken look around my eyes—especially near the orbital bone—and I’m not sure what’s contributing to that. I’d really appreciate your opinion on whether a brow lift, upper or lower blepharoplasty, fat transfer, or something else might help. Also wondering if a full facelift is something to consider now, or if that’s better down the road. My main goal is to look more rested and refreshed, while keeping things natural.

Your blepharoplasty is pulling down your eyebrow. Further blepharoplasty will only make this worse.
You need a browlift to restore the previous height of your eyebrows.

There are patients who have a genetic predisposition to having lower eyelid bags at an early age. This can easily be corrected with a quick lower eyelid surgery to smooth them out. And yes, if it's obviously present, it's OK to do it in your 20's. Be sure to see a facial specialist for consultation.
