My nose was flatter before and my surgeon built it up with rib conservatively. I asked my surgeon to defat the part below the tip if possible. I drew a black dot where the tip is. I’m referring to the part where the columella meets the bottom of the tip where I drew the red arrow (not the tip itself). He said it’s too dangerous to thin this part out. Is it possible to reduce or thin it a good amount? I added a photo of what I’m referring too.

Indeed, the tip looks a bit wide to me, and not all that dissimilar to the photos you posted here before your primary. That tends to suggest that not enough was done to narrow your tip, and perhaps more can be done. Everything gets harder after two rhinoplasties. I'm wondering why you're in this...
Your surgeon was partially correct. When you have that degree of alar retraction, you do need additional skin order to bring the nostril notching downward. It is possible to do but it is not a commonly performed procedure so most likely your surgeon did not tell you about it, esp if it was...
Based on your photos, you appear to have relatively thin nasal skin and congenital indentations of your tip cartilages on both sides. We have had excellent success in filling these indentations, when they occur, with microdroplet Silikon-1000. I hope this helps, and thank you for your question....
